feat: multi-channel notifications - Discord, Slack, email (Closes #3) ... 9656633a31

- notify setup <type> [--url|--email] — configure channel
- notify list — show channels
- notify remove <type> — remove channel
- notify test [--channel <t>] — test notification
- Discord/Slack webhook + email SMTP support
- Per-channel message templates
- 14 new notify tests, 96 total passing
