feat(#1): PR fortune engine — predictions, crystal ball, review omens ... 265bef9462

- src/fortune.js: Full fortune teller engine
  - 6 PR outcome predictions with probabilities
  - 15 omens (good/neutral/bad)
  - 12 fortune cookies for devs
  - Risk modifier system from PR attributes
  - Confidence percentage (entertainment only)
  - ASCII crystal ball rendering
- 25 tests
